If you are planning to go abroad and need travel vaccines please print this Travel Form and make an appointment directly with the nurse. Please give yourself plenty of time around 4 weeks before travelling.
If you are newly registered and not seen a doctor before, we recommend you see one first to discuss any previous medical history you may have. The doctor will not be able to give you the travel vaccines but you can book a nurse's appointment afterwards.

Family Planning and Emergency Contraception
Your doctor will be able to advise you about either of the above. Please make an appointment. The Duty Doctor will be able to advise you regarding the morning after pill.
If you are diabetic your doctor will invite you to our clinic unless you are already under regular hospital care.
It is recommended that women have this test every 3-5 years dependent upon your age. Please book an appointment with the practice nurses.
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

To help us offer the appropriate advice, please fill out the online form and print out a copy. You will need to book an appointment with the nurse and bring your completed questionnaire with you to your appointment.
Travelling in Europe
If you are travelling to Europe the EU has published useful information for travellers on the European website.
Some services provided are not covered under our contract with the NHS and therefore attract charges. They are listed in the table bellow with their respective fees, based on the British Medical Association (BMA) suggested scales.

Long Acting Reversible Contraception
Intrauterine Devices (copper coils ) and Intrauterine Systems (IUS /mirena ) are able to be fitted at the surgery. Please discuss with a Dr / Nurse
Implants (nexplanon) are able to be fitted (as well as removals) again please discuss with a doctor / nurse if you are interested in the above methods.
