Hypertension annual review

Use this service if you have been advised by the surgery to submit an annual review of your high blood pressure.

A healthcare professional at the practice will review your form. If your blood pressure reads over 170/115 and is still this high when you re-check it a few minutes later, please call the practice for an urgent same day review.

High blood pressure (hypertension) happens when the force on the walls of blood vessels (caused by the blood within them) is more than normal. This means the heart has to work harder and the blood vessels are under more strain, which means there is increased risk of heart disease and stroke.

Your blood pressure reading will have two figures, one ‘over’ the other – for example, 140/80. The higher figure (called the systolic level) is a measure of the pressure inside your arteries when your heart is pumping blood out. The lower figure (called the diastolic level) is a measure of the pressure inside your arteries when your heart is resting between beats.

  • are registered at the surgery
  • have been invited to do so

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
