Temporary patient registration

Use this service if you need to be registered as a temporary patient.

If you have a friend or relative staying with you for up to three months who becomes ill, they may be seen as a temporary resident during their stay in the area.

We will also see anyone who needs treatment whilst they are staying in the area on holiday.

You can use this service if you:

• want to register at the surgery temporarily
• are temporarily staying within our catchment area

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• details of previous GP surgery
• basic health and medical information

We would request that you provide a personal and secure email address. If you share your email with anyone else, they will be able to see confidential communications from the practice. Please see our guidance on safe online access to your medical records for more information.
